Application for Member-Owners

Cooperatives are businesses owned by “member-owners”. Co-ops are democratically controlled by their member-owners, and unlike a traditional business each member gets a voice in how the business is run. Services or goods provided by the co-op benefit and serve the member owners. Applications for member-ownership are open to doulas who serve families in Rhode Island and:

  • Certify as a Certified Perinatal Doula with the Rhode Island Certification Board within one year of application.

  • Become a Medicaid provider and accept Medicaid insurance within one year of application.

  • Pay an annual fee of $35. This will be invoiced annually to the applicant should the board of directors approve the application.

  • Sign a non-disclosure agreement
